    Coaching Agreement​

    This coaching agreement is between: Life, Love & Parenting - Julie L Gibson-Vasquez ​ (The Coach) and​
    The coach will provide the client coaching services. As designed jointly with the client, coaching, which is NOT advice, therapy or counseling, may address specific personal, parenting or family matters, personal or business goals, and/or general conditions in the client’s personal, family, and professional life. Coaching may include value clarification, brainstorming ideas and identifying plans of action, examining the client’s model of operating in life, asking clarifying questions and making empowering requests.

    The coach guarantees to work for the best interest of the client at all times. When there is a conflict of interest, the coach will always put the client’s interest first. In the unlikely event that this is not possible, the coach will inform the client and the parties will agree a course of action.

    Coaching will be provided at mutually agreed sessions, face-to-face, over the phone or via Skype/the Internet. The coach will also answer the client’s questions via email in-between sessions for the duration of the coaching relationship (replies within 48 hours). The client takes full responsibility for any decisions made during coaching and agrees to NOT hold the coach liable for any losses, or any condition incurred by the client, directly or indirectly relating to the client’s coaching sessions. The client takes full responsibility for the achievement of goals and will NOT hold the coach liable for any goals not achieved. The client agrees to provide the coach a minimum of 24 hours’ notice if a session cannot be attended. If the client does not give notice within that time, the client will pay a cancellation fee of $50. All payments must be made at time of scheduling or the coach may terminate the agreement.

    The coach guarantees that all information disclosed by the client during coaching sessions will be kept strictly confidential.

    The client agrees that the information provided during the coaching is for personal use only.

    The Life, Parent & Homeschooling Coaching fee is $65.00 (US Dollars) per individual 1 hour session and $99.00 for 2 hour sessions. Couple’s & Family 2 hr session fee is $150. Most 6-session programs, unless otherwise stated, have a fee of $350.00 for all sessions. (50% off online coaching and any other periodic discounts shown when you scheduled your appointment will be honored.)

    Required: If you have not already done so, please create an account on the Life, Love & Parenting appointment scheduling website @ https://lifeloveparenting.setmore.com. Your account will allow you easy access to making new appointments, receiving appointment changes, reschedules or cancellations, paying invoices, and receiving appointment reminders.

    The Loyalty Program: The coach will pay the client $25 for each new client referred, once the new client has paid in full for their first coaching session.

    By type-printing your name in the signature box below, and submitting these forms using the SUBMIT ALL button at the bottom of this page, you are agreeing to the terms of the contract and the contract will become valid upon submission. Submission of this filled coaching agreement, along with assessments, indicates full understanding of, and agreement with, the information outlined above.
